Case Management Case management offers professional support in managing HIV/AIDS to ensure quality and comprehensive care. This is accomplished by providing and/or accessing community services to maximize health, reduce stress, and improve the overall quality of life of APQC clients. The client population served by APQC includes both women, men, and children with ages ranging from preschool to senior citizen, of varying ethnic backgrounds. While most are employed, many clients are underinsured or uninsured altogether. HIV/AIDS disproportionately affects the low-income population, placing further financial burden on those with too few resources. Many lack transportation and most are unsure how to access government assistance programs. As medical advances add to the quality and length of life for our clients, there is a growing need for supportive services .The treatments for HIV/AIDS can be complicated and time-consuming. A stable environment is crucial to successful treatment. Case managers help to create that stable environment by offering access to services which assist with medical expenses for uninsured clients, provide housing and utility assistance for low-income clients, and assistance in returning to work when clients are able to do so. Clients with mental health and substance abuse issues are connected with agencies and programs suited to their needs. In short, case managers working in conjunction with health care professionals provide clients with the resources necessary to get healthy and stay healthy.
